Not only can you transmit your vital sign measurements through the PAT but you can ask specific questions regarding your health issues and protocols to take action, as well as take surveys regarding your individual programs. How are the question responses captured?
Each patient completes a 5-10 minute daily session in which they respond to a series of questions pertaining to their health condition. The responses are then transmitted to the VITAL Telehealth Clinician Portal where a health professional can review the information and follow up with the patient if needed.
