The PAT uses Extensible Markup Language (XML) files to provide patients with interactive audio content about specific health programs or diseases. Not only can you transmit your vital sign measurements through the PAT but you can ask specific questions regarding your health issues and protocols to take action, as well as take surveys regarding your individual programs. Providers can immerse their patients in an engaging experience in the management of their healthcare. The PAT provides comprehensive patient education regarding their health.

Not only will the PAT provide you with the status of your surveys but will update you with your progress, survey start and end dates, and new information since the last time you logged onto the PAT. After the patient has answered and submitted their surveys, providers and clinicians can view the information and follow up with patients as needed.

  • How are the question responses captured?

    Each patient completes a 5-10 minute daily session in which they respond to a series of questions pertaining to their health condition. The responses are then transmitted to the VITAL Telehealth Clinician Portal where a health professional can review the information and follow up with the patient if needed.

    Yes. VITAL PAT can support patients with multiple health conditions by integrating different combinations of our chronic disease management programs.
