Who are we?

The VITAL Telehealth public website (https://www.vitaltelehealth.com), the VITAL Telehealth secure web portal environment, the VITAL Telehealth iOS apps, the VITAL Telehealth Android apps, and the VITAL Telehealth Windows apps were developed and are currently maintained by the VITAL Telehealth software development team, which is a division of Govsphere, Inc. (our parent company).

What information do we collect?

We collect two (2) types of information: user-provided information and automatically collected information.

User Provided Information

The VITAL Telehealth public website, secure web portal, iOS & Android mobile apps, and Windows apps obtain information that you provide when you complete the initial registration for an end user account. It is important to note that completing the registration process is optional. However, please keep in mind that you may not be able to utilize the features offered by our VITAL Telehealth platform, our websites, our mobile apps, or our Windows apps unless you have completed the registration process.

When you complete the VITAL Telehealth registration process, you generally provide (a) your full name, mobile phone number, email address, age, username, password and other registration information as required to fully utilize the features offered by the VITAL Telehealth websites, iOS & Android mobile apps, and Windows apps; (b) transaction-related information, such as when you make purchases, respond to any offers, or download or use applications from us; (c) information you provide us when you contact us for technical support or assistance; and; (d) information that you enter into our system when utilizing VITAL Telehealth services including, but not limited to: contact information, demographic information, and app-specific information. It is important to note that you must be at least 12 years of age in order to create a VITAL Telehealth account.

Furthermore, we may also use the information that you provided us to inform you of software updates and security patches along with important information, required notices and marketing promotions. You have the option to opt-out of these periodic communications at any time.

Automatically Collected Information

In addition, the Application may collect certain information automatically, including, but not limited to: the type of mobile device you use, your mobile device’s unique device ID, the IP address of your mobile device, your mobile device’s operating system, the type of mobile Internet browsers you use, and information about the way you specifically use the VITAL Telehealth website, iOS & Android mobile apps, and Windows apps. In the event of an app crash, we may also collect diagnostic information from the VITAL Telehealth application logs in order to diagnose any potential bugs or performance-related issues.

Media

If you upload images to the website, we may collect and maintain this information within your profile. It is important to note that you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images uploaded to the website.

Cookies

If you leave a comment on our VITAL Telehealth public website or secure web portal, you may opt-in to saving your full name, your username, email address, and/or website in cookies. These cookies are offered for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one (1) year. However, you may also remove the cookies at any time by deleting the cookies.

If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser. When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for one (1) year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.

Who do we share your data with?

Internal Data Access

If you need technical support, such as a password reset or the creation of a new account, your information will be accessed by members of our VITAL Telehealth support team as required in order to satisfy your request.

Third-Party Data Access

We may disclose User Provided and Automatically Collected Information to third parties:

  • as required by law, such as to comply with a subpoena, or similar legal process.
  • when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request.
  • with our trusted services providers who work on our behalf, do not have an independent use of the information we disclose to them, and have agreed to adhere to the rules set forth in this privacy statement.
  • if our business entity is involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified via email and/or a prominent notice on our Web site of any change in ownership or uses of this information, as well as any choices you may have regarding this information.

How long do we retain your data?

If you leave a comment or submit information through the VITAL Telehealth public website, the VITAL Telehealth secure web portal, the VITAL Telehealth iOS and/or Android mobile apps, or the VITAL Telehealth Windows apps, the submitted information and its metadata are retained as long as your VITAL Telehealth account is active. If you choose to delete your VITAL Telehealth account, then your data and information related to your account will also be deleted.

For users that register on our VITAL Telehealth public website or VITAL Telehealth secure web portal, we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

What rights do you have over your data?

If you have created a VITAL Telehealth account, you can request an exported file of the information that you have submitted through the VITAL Telehealth public website or the VITAL Telehealth secure web portal linked to your account including any data you that have provided to us. You can also request that we erase any personal data we hold about you upon deleting your VITAL Telehealth account. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

What are my opt-out rights?

You can stop all collection of information by deleting your VITAL Telehealth account and uninstalling any VITAL Telehealth mobile apps that you have previously installed. You may use the standard uninstall processes as may be available as part of your mobile device.
